Review and complete ALL of the intake documentation that is sent to you before the first session. If you have any issues completing it, reach out by phone for assistance.

Upload a pic of the front of your government issued ID.

Upload a pic of the front and back of your insurance card

When You Arrive…
For in-person sessions, note that the front door remains locked between sessions and your counselor will open the door for you at session time.
There is a restroom available for clients, bottled water and coffee and tea upon request.
Please be quiet and respectful of others while in the office building.
